Day Care Report: Academy cited for live roaches

7 facilities hit with violations in recent DCF inspections

JACKSONVILLE, Fla. – More than a dozen local day cares were inspected recently by the Department of Children and Families and seven were hit with violations.

News4Jax dug through the reports from DCF and visited one of the day cares to make sure it is keeping children safe.

Kreative Minds Academy on Edgewood Avenue North in Northwest Jacksonville was cited for a bug problem.

According to the inspection report, a DCF counselor found several live roaches in the kitchen cabinets where utensils and pans are kept.

News4Jax spoke to one of the staff members at the day care who believes the roaches came from a donated box of apples that had been placed on the cabinet.

Since the inspection, the day care has had an exterminator treat the area, and now they have pest control come once a month while the kids are away.

Last week, Wayman Early Learning Center made the News4Jax day care report card after receiving several violations. It was put on a provisional license for not having a certified director, but Pastor Mark Griffin of Wayman Ministries said the academy has been open for 20 years and has served more than 1,000 kids.

In a statement to News4Jax, he said: "When a child care center loses it director, it is standard procedure for DCF to place the center on provisional license status until a new credentialed director is hired. We have two credentialed individuals currently on staff, and are in the process of hiring a third such individual. Once we determine which person is best qualified to serve as the director of record, we will submit that information to DCF, which will remove the provisional status.  None of the findings in the report were health, safety or welfare concerns, which show the quality of care we provide to our children. The findings were administrative in nature, and reflect the fact that we are in transition with our administrative staff."

DCF said Wayman Early Learning Academy has until the end of August to have a credentialed director registered with the state, otherwise its license will be revoked.

Day cares with recent violations:

Kreative Minds Academy Inc. on Edgewood Avenue North

  • Counselor observed several live roaches in the kitchen cabinets where utensils and pans are kept. 
  • Counselor observed all purpose spray cleaner accessible to children on the cabinet in the two-year-old room.

Burrell's Child Care Center Inc. on Spring Park Circle

  • Areas of facility were found to be in need of cleaning. The door has peeling paint that needs to be re-painted.
  • Door must be repaired. There is a large gap in the door that could allow animals and bugs into the facility.
  • Exit areas blocked off and inaccessible in an emergency.
  • Fire drills were not being conducted and di dnot include one drill during napping times. The last drill was recorded on March 16.

A Little Preschool on Sunderland Road

  • Shot records missing for children
  • Abuse and neglect form for three employees expired. All employees signed new form during inspection.

Lakewood United Methodist Preschool on San Jose Boulevard

  • Staff member needed to complete Introductory training by March 12, which was 15 days from date she began training. She began training before date of hire in industry. Needs to complete training and pass test.

Beverly Hills Development Center on Rogers Avenue

  • Staff member needs to complete training course.

Little Temples Child Care Center 1 on Faye Road

  • Two employees' training transcripts have not been updated with the current credential documents.

Kiddieville Christian Academy

  • Employee needs to complete early literacy course.
