MACCLENNY, Fla. – A woman who the Florida Highway Patrol says caused a crash last fall on Interstate 10 that killed a youth pastor from Baker County has turned herself in on charges of careless driving and driving on a suspended or revoked license.
Holly King, 22, surrendered Friday to charges resulting from the Sept. 16 crash that killed 32-year-old Todd Annis.

Annis led student ministries at the First Assembly of God in Macclenny and also volunteered as chaplain of the Baker County High School football team.
According to the FHP, King's car sideswiped Annis' car as they were both westbound on I-10 near the Chaffee Road interchange. Annis' car spun out of control and overturned. Annis was ejected, pinned under the car and died. His wife survived the wreck.
King was not hurt.
FHP Lt. Bill Leeper said the arrest was delayed because King was not considered a flight risk and investigators were waiting on the result of toxicology testing before filing charges.
The tests found no evidence of alcohol or drugs.
Prior to the September crash that killed Annis, King had 11 speeding violations -- once going 93 mph in a 60 mph zone -- one for careless driving and one for driving with a suspended license. He had been pulled over for traffic violations in seven counties.

King has posted $25,000 bail and was released from jail.
Annis spent a year and a half commuting from Jacksonville to Macclenny to be a youth pastor. Even though he didn't live in Baker County, that's where his heart was, and he was such a huge part of the community.
"I think that's what I miss the most is him just being able to wrap his arms around me and tell me everything's going to be fine," Annis' widow, Lauren Annis, said.
She wrapped her arms around a picture taken two hours before the crash. Lauren and Todd were leaving after watching a Gators football game with their youth group when the crash happened.
"He was my best friend and he was the person I told everything to," Annis said.
"Todd was very unique. He had so many solid strengths, wonderful personality, very caring, very genuine," the church's pastor, Josh Potts, said.
Annis hopes the accident can serve as a reminder to others that the wrong decision to get behind the wheel when you're not supposed to be can affect so many lives.
"She made that choice to drive that night. She made the choice to drive and to drive recklessly," Annis said of King. "And in result, my husband lost his life, a community lost their youth pastor and an influential person. And if I could say anything, just be cautious of what you're doing and the choices that you make."
It's a night Annis will have to relive for the rest of her life as she carries on her husband's torch.
For the past five months, Todd's wife has carried on his legacy as the youth pastor of the church. Potts said the students leaned on her just as much as she leaned on them.
