No purchase necessary. Winner to be chosen at random. Must be a legal U.S. resident 18 years and older, with a valid Social Security number. Winner will be required to sign a release of liability and W9 Tax Form upon winning.Employees (and relatives) of WJXT-TV or Incentive Plus Network are not eligible to win. Only residents of the Jacksonville television DMA are eligible to win. Those counties are Duval, Nassau, St. Johns, Clay, Baker, Putnam, Bradford, Union, Columbia, Charlton, Ware, Pierce, Brantley, Camden and Glynn.Grand prize is a 42-inch high-definition, plasma screen TV. The make, model, color and other features of the television are not negotiable. Prize Winner is solely responsible for all applicable taxes, including but not limited to, installation charges and/or additional hardware costs.The entry process is as follows: The first clue will be given out Sept. 18, 2006, during the Monday Night Football game on Channel 4, then two more each night from Sept. 19-22 between 9:30 and 10:30 p.m. Clues will only be given out once. There will be a total of nine clues.After the final clue is given out on Sept. 22, viewers will be prompted to call in with the answer to the puzzle. If the fourth caller has the correct answer, they are the winner, provided they meet all necessary guidelines. If the fourth caller does not have the right answer, the fifth caller will be taken, then the sixth until a qualified caller gets the answer right.The correct answer is at the sole discretion of WJXT. By participating in the contest, you agree to the terms of entry.The date of distribution of this prize is not guaranteed and is at the sole discretion of Channel 4.Additional Conditions
- Payments of all federal, state and local taxes are solely the responsibility of the winner. Winner may be required to sign an IRS Form W-9 or the equivalent. The value of the prize will be made known to the winner prior to taking receipt. The winner has the option of refusing the prize if they choose, but by doing so waives WJXT-TV and all other associated parties from any future responsibilities.
- By participating in the contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest, in perpetuity, without additional financial or other compensation, and where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
- Prior to awarding any prize or prize certificate, WJXT-TV at its sole discretion may require contestant to sign a liability release, agreeing to hold WJXT-TV, its corporate licensee, its parent corporations, affiliated companies and the officers, shareholders, directors, and its owners and employees harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest.
- WJXT-TV reserves the right to cancel, terminate or modify the contest if it is not capable of completion as planned due to political, catastrophic or any other conditions throughout the world that prevent award of this contest.
- WJXT-TV reserves the right to make changes in the rules of the contest, including the substitution of a prize or equivalent value, which will become effective upon announcement.
- Failure to comply with the contest rules may result in a contestant’s disqualification, at the sole discretion of WJXT-TV.
- WJXT-TV is not responsible for typographical or other errors in the printing, the offering or the administration of the contest, or in the announcement of a prize.
- WJXT-TV reserves the right to add or amend the rules of this contest until the date of the prize distribution.
- Winner is responsible for taking delivery of the prize and all liability associated with its transportation.
