JACKSONVILLE, Fla. – The Florida Department of Health in Duval County has launched its "Go
to the Head of the Class" campaign to help parents and families with immunizations and physicals as they prepare for the upcoming school year.
The health department is offering immunizations for students leading up to the school year at the Duval Immunizations center at 5220 North Pearl Street Monday through Friday.

The hours are: 8 a.m. to 4:30 p.m.
On the 2nd and 4th Wednesday of the Month, immunizations will be available from 8 a.m. to 12 p.m.
Walk-in School Physicals (not sports physicals) are also available at the center.
Kindergarten through 12th grade
July 21 – August 20
Tuesdays and Thursdays
8 a.m. to 3 p.m.
$30 per exam.

Current DOH-Duval parents are encouraged to call 253-1130 to schedule an appointment where their child is usually seen.
It is always best to have a complete physical done by the doctor who knows your
child.
