The FHP report did not indicate how the crash happened, other than it was a chain reaction involving four cars. A tractor-trailer may have also be involved. Troopers say Melissa Wenth, 35, from Jupiter was ejected from her car. She was taken to UF Health Jacksonville, where she later died.

Troopers say those who suffered minor injuries in the other vehicles were a 31-year-old man named James, Hector Berrio, 60, Sarabeth Mach, 30, Laila Mach, 10, Paityn Mach, 5 and Mia Mach, 1. They were all taken to Flagler Hosptial.

FHP says that the driver and passenger of a Toyota Corolla were not at the scene by the time officials arrived.

The crash and investigation kept all northbound lanes of I-95 closed for four hours.
