Woman killed in crash at entrance to I-95 in Yulee

FHP: Charges pending results of blood-alcohol test

YULEE, Fla. – A 68-year-old woman died overnight after a pickup truck collided with a tractor-trailer turning left from State Road 200 onto the entrance ramp to Interstate 95, according to the Florida Highway Patrol.

Around 11:50 p.m. the big rig driven by 43-year-old Phillip Lyons was attempting to turn onto southbound I-95 when it collided with the front end of a Chevrolet S-10.

According to FHP, the tractor was making a turn on a yellow flashing turn arrow, when the pickup was traveling the other direction on S.R. 200.

The pickup was driven by 21-year-old Jimmy Fralick III, with 68-year-old Martha Fralick in the passenger seat. Both Jimmy and Martha were transported to the UF Health Jacksonville, where the passenger later died.

Phillip Lyons, driver of the Mack tractor, sustained minor injuries as a result of the crash. Jimmy Fralick's injuries were also reported to be minor.

Lyons volunteered to give blood for testing. According to the FHP report, charges are pending the results of that test and its traffic homicide investigation.
