Day care inspections find unlicensed staff, unsafe equipment

JACKSONVILLE, Fla. – Unlicensed day care staff, unsafe playground equipment and expired vaccination records are a few violations counselors with the Department of Children and Families found during their recent inspections of local day cares.

Many of the nearly two dozen day cares inspected recently are fully compliant with DCF standards. But some were not following the rules during their most recent inspections.

Bosman's Family Academy of Learning in Arlington had five violations after a routine inspection. According to the report, two newly hired staff members did not have background check or employment forms on file.

The day care director was able to gather those forms during the inspection but found out that one of those new employees did not have proof that she completed the Child Care Training required by DCF. Bosman's director, Larry Bosman, said that she was fired after lying about some of the training that was not on file.

The day care was also cited for not having enrollment forms for six children, and that problem was corrected during the inspection.

The Academy of Success Learning Center in Arlington received three violations during its last inspection.

When counselors showed up, there was no available staff member with CPR or first aid certification at the facility. According to DCF rules, at least one staff member must be certified in CPR and first aid.

Director Maclisa Combs said she is certified but was out picking up kids during the inspection. Although only one staff member needs to be certified, Combs said all of her staff members are scheduled to get certified in CPR and first aid.

The report went on to say that two kids at the day care had expired shot records. Combs said one child is now up-to-date and the other has an appointment to get the required shots.

Little Saints Child Care in the Spring Park area received six violations during its last inspection.

The counselor noted that the ropes on the swing set pricked the hands of the person swinging and need to be replaced. The report also said that the "fall zone" under the playground equipment needed to be maintained so the kids had a safe, protective surface in case of a fall.

Little Saints was also cited for a few paperwork issues, like not posting emergency numbers near all of the phones or posting the facility's license, but those violations were corrected during the inspection.

The numbers listed for the facility have been disconnected.

Day cares with violations:

?Little Saints Child Care -- 2179 Emerson St. (Jacksonville)

  • The facility's license was not posted within the facility
  • A spray bottle of Lysol Disinfectant was in an unlocked bottom cabinet in the infant's room. The provider removed it from the cabinet and placed it on the shelf above the sink. 
  • The swing rope on the small children playground pricks the hands of the person swinging. These ropes need to be replaced.
  • Diaper changing occurred on a surface that was not impermeable. The changing mat in the infants' room was torn. The provider replaced the torn mat.
  • The fall zones under the yellow slide, green slide, circular bike wheel and both sets of swings need to be maintained.
  • There are no emergency phone numbers posted near all phones. The provider posted emergency numbers by the phones during the inspection.?

Bosman's Family Academy of Learning Inc. -- 6240 Fort Caroline Road Suite 1-4 (Jacksonville)

  • Based on log posted on the wall, the last fire drill completed was for January 2015. Fire drills must be conducted monthly. A fire drill was completed during the inspection, at the counselor's request.
  • Staff member started in child care industry on 3/8/2010, but to date she has not completed the required 40 hours of training.
  • 6 children did not have enrollment forms on file.
  • 2 newly hired staff members did not have employment application on file. 
  • 2 newly hired staff members did not have background screening and personnel file on hand at the day care. Forms were given to the director and completed at the time of inspection.

Academy of Success and Learning Center -- 6221 Merrill Road (Jacksonville)

  • The facility did not have at least one staff member with current first aid training certification present at all times that children are in care.
  • The facility did not have at least one staff member with current CPR training certification present at all times that children are in care.
  • 2 children were missing their immunization certificates.

Little Wise Kids Early Learning Center PLLC -- 5571 Playa Way (Jacksonville)

  • Staff member did not complete five hours of early literacy training within 12 months of her start date in the child care industry.

Little Fountains of Light Learning Center -- 7928 Rampart Road (Jacksonville)

  • Staff member's Child Abuse and Neglect Reporting form was expired. Form was completed and filed at the time of inspection.
