NASSAU COUNTY, Fla. – A 20-year-old Georgia woman died Saturday afternoon in a single-vehicle crash in Nassau County, the Florida High Patrol reported. According to FHP, the Augusta woman was traveling eastbound on State Road 200 around 2 p.m. when, for reasons still being investigated, she traveled off the roadway to the right and over a raised concrete curb in the median. After traveling over the median, her SUV overturned and landed in a retention pond on the entrance ramp to Interstate 95, FHP said. No other details, including the driver’s identity, were immediately available.

ST. JOHNS COUNTY, Fla. – A 17-year-old Clay County boy was killed Friday night in a head-on crash on State Road 16 at Francis Road in St. Johns County, according to the Florida Highway Patrol. The Middleburg teen was a passenger in a car driven by a 16-year-old boy, also from Middleburg, the FHP report showed. According to the crash report, the 16-year-old driving on State Road 16 about 8:15 p.m. Friday ignored the right of way of a pickup truck at the intersection with Francis Road. The resulting head-on crash killed the 17-year-old passenger of the car and seriously injured the 16-year-old driver. All lanes of SR-16 between I-95 and Pacetti Road were blocked in both directions while troopers investigated the crash.

ST. JOHNS COUNTY, Fla. – A St. Augustine memorial that honors 46 Confederate soldiers has finally been erected at its new home at the Trout Creek Fish Camp in western St. Johns County. It will now sit in front of a pavilion on the fish camp grounds. The contractor hired by the city told News4Jax another layer of concrete will be poured around the memorial Thursday to complete the project. Controversial decisionIn June, the St. Augustine City Commission voted 3-2 in favor of moving the Confederate monument that was built in 1872 in downtown St. Augustine. A statue of Confederate Gen. William Loring that was located feet away was also removed last month.

The Jacksonville Sheriffs Office says a semi truck overturned on the exit ramp from State Road 9B to I-95. Two traffic lanes have been blocked in the incident. The Florida Highway Patrol says at least one person was hurt in the crash, but have not said what caused it. JSO has yet to say when the road will reopen. Motorists driving through that area are urged to use caution.

ALACHUA COUNTY, Fla. – A Gainesville woman was killed and two others were injured in a single-vehicle crash Saturday night in Alachua County, according to the Florida Highway Patrol. The Highway Patrol said a van was traveling east on State Road 20 around 10:30 p.m. when it went off the road and collided with the tree line. Troopers said one passenger, a 31-year-old Gainesville woman, died at the scene. The Highway Patrol said the other passenger, a 33-year-old Gainesville woman, suffered minor injuries and the driver, a 28-year-old Gainesville woman, suffered serious injures. According to the Highway Patrol report, none of the three occupants were wearing seat belts.

SAN MATEO, Fla. One person died in a crash Monday involving a semi and a vehicle in San Mateo, according to the Florida Highway Patrol. The crash was reported about 10:30 a.m. on southbound U.S. 17 at State Road 100. At last check, the right lane of southbound U.S. 17 was blocked. Troopers asked drives to avoid the area if possible.

LAKE CITY, Fla. – Video taken from a Columbia County deputy’s cruiser shows the EF0 tornado that touched down in near Lake City over the weekend. According to the National Weather Service, the tornado hit State Road 47 around 3:30 p.m. Sunday. PUTNAM COUNTY, Fla. – A woman sought in a hit-and-run was located by the Florida Highway Patrol, the Putnam County Sheriff’s Office announced Sunday. The Sheriff’s Office said the hit-and-run took place about 5 p.m. Friday at Fast Track on State Road 20 and Sun Avenue. Deputies said another vehicle was damaged. On Saturday, the Sheriff’s Office shared photos on its Facebook page of a woman and asked for the public’s help to identify her in connection with the case. Less than 24 hours later, according to the Sheriff’s Office, the Highway Patrol was able to find the woman pictured thanks to tips.

ALACHUA COUNTY, Fla. Three people, including a husband and wife from St. Marys, were killed and the couples two children were injured in a head-on crash Saturday night in Alachua County, the Florida Highway Patrol said. According to the Highway Patrol, a Hyundai Elantra was traveling westbound in the eastbound lanes of S.R. Troopers said the driver of the Toyota -- Christopher Hesling, 30, of St. Marys, Georgia -- and one of the passengers -- Brittany Hesling, 31, of St. Marys -- died. The Highway Patrol report shows Brittany Hesling was not wearing a seat belt. The pair most recently lived in a neighborhood near downtown St. Marys with their two children.

FRUIT COVE, Fla. - Met with much opposition from neighbors, St. Johns County commissioners voted Tuesday against a plan to build a gas station in Fruit Cove. The Daily's 24-hour gas station was planned to sit along State Road 13 on a nearly 8-acre lot outside of Fruit Cove Estates. The developer argued Tuesday that a gas station in the area would be better than the shopping center the property is currently zoned for. Applicants previously told News4Jax the new gas station would be a good economic decision for the area, supporting a boom in population. Neighbors also fought against the gas station in 2016, and commissioners then voted against the proposal.

FRUIT COVE, Fla. - It was an emotional day for many who spoke out Thursday against a Daily's 24-hour gas station being built along State Road 13. The nearly 8-acre plot of land sits just outside Fruit Cove Estates. "Daily's told us that a gas station here is going to be the greatest thing that ever happened to us, we didn't drink the Kool-Aid," Fruit Cove resident, Harry Fairfield, said. "There are 18 properties within 300 feet of this gas station. These are our families, and to be honest, we feel violated," said Cindy Dunlop, a Fruit Cove resident.

O'NEIL, Fla. - A driver and her passenger were not hurt Monday afternoon when a car crashed into a retail store under construction on State Road 200 between Yulee and Fernandina Beach, according to the Florida Highway Patrol. According to the FHP report, Jana Laddusaw, 41, of St. Marys, Georgia, was driving a 2009 Cadillac DTS that crashed into the storefront of what was previously Pinch-a-Penny and the Smokehouse Meat Market about 1:10 p.m. The passenger was a 63-year-old woman from Fernandina Beach. JOHNS COUNTY, Fla. - A traffic alert for drivers in the Switzerland area of St. Johns County: Fallen trees and power lines have left State Road 13 closed in both directions just south of Greenbriar Road on Sunday. At last check, this was still an active scene, with five or six work trucks set up behind the work zone cones. Multiple downed power lines were seen resting on bushes along the side of the road. Our team saw a pretty large tree down on the river side of State Road 13, as well. The St. Johns County Sheriffs Office said the road will be closed for several hours.

GLEN ST. MARY, Fla. - A Baker County man was charged with second-degree murder after a 36-year-old woman was struck and killed while walking along a highway south of Glen St. Mary, according to the Florida Highway Patrol. When the driver was located at a home less than a mile from the scene, Baker County deputies learned he knew and apparently targeted the victim. When Baker County deputies questioned Jackson, he said he suffered from schizophrenia but had been off his medication for two years. LAKE CITY, Fla. – A 25-year-old Lake City woman was hospitalized in critical condition early Tuesday after a rear-end collision in Columbia County, according to the Florida Highway Patrol. Troopers said a sport-utility vehicle driven by Alyssa West was heading south on State Road 25 about 7 a.m. when it struck another vehicle traveling ahead of it. West, who was not wearing a seatbelt, was taken to UF Health Shands Hospital in Gainesville to be treated for critical injuries, troopers said. The other driver, 64-year-old Pierre Avesca, was not hurt.

STARKE, Fla. – A tractor-trailer car carrier pulled down wires and several power poles along State Road 100 Thursday afternoon, the Starke Police Department said. SUWANNEE COUNTY, Fla. – A woman suffered serious injuries after her truck rolled over Tuesday on State Road 10, a half-mile west of County Road 137. The Florida Highway Patrol said witnesses reported the woman, identified as Bonnie Coy Omalley, 47, was driving east on SR 10 when she veered off the road into the grass. She attempted to correct, but that caused her vehicle to begin spinning and travel off the road again, onto a grassy shoulder. Troopers said Omalley's truck began to flip and she was thrown from the vehicle. Omalley was taken to UF Health Shands Hospital as a trauma patient.
